How can we offer this without affecting quality of care?
The same way your family veterinarian does! A lot of these surgeries are performed by general practitioners; when it makes sense to do so (ie/ early enough in the day to allow recovery to go home, or for transport to a referral hospital for overnight and continued care). It just happens that we are also here overnight, fully staffed, because of our medically sick patients. And, because, we do these surgeries on a regular basis, we are fully equipped and fully staffed, 24/7. We are not doing surgeries that require a boarded surgeon, an anesthesiologist, or ICU care with a criticalist. They have years of specialized training, and more specialized equipment (ie/ CT scanner, ventilator etc.).
